Paving slab proud causing trip

Date and time of incident: Thursday 1st April early afternoon about 14:00. My wife & I were on a daily local walk when she tripped on a proud, badly aligned pavement slab, see photo. She fell forwards hitting her head on the pavement breaking her glasses, see photo, and cutting open above her left (L) eye enough to need 4 stitches at the 'Urgent Care Centre' at Queen Marys hospital, see photo. I did copy over a photo of the blood spatter on the pavement but it seems not to be there, I think you only accept 3 photo's. The incident happened on the east side of Montrose Avenue walking towards Burnt Oak Lane , Sidcup, outside No. 30 Montrose Avenue.
